I've worked for large organizations and I can assure you that this type of thing is commonplace with such organizations. There are a LOT of button pushers who get employed for whom, this might cross their desk, they have no idea what to do about it - nor do they have any accountability to do so - so they do nothing.

My nickel bet here is, it is less about seemy dishonest culture .. which the Commanders definitely have! Rather, this is sloppy inefficiency culture.
